General claims information
The best place to start a claim is on your affinity group’s website. Once there, select Submit a claim. Here’s some general claims information you can also refer to:
Many hospitals, pharmacies and dentists send claims directly to us. There’s nothing for you to do, unless there’s a balance owing. If there is, you would simply pay the part your plan doesn’t cover. Then you’re done.
If your provider hasn’t already sent your claim in, you can submit it yourself online or by mail.
Online: Sign in to secure serve to start your claim.
By mail: Find the correct form, fill it out, and be sure to sign it. Send your signed form to the address printed right on the form.
For both online claims and mail claims, please note the following:
- Submit your claim within 12 months of when you were charged.
- After you’ve paid your deductible, we’ll reimburse you for any outstanding balance.
- If you had the service while you were outside of Canada, tell us the currency you paid the bill with.
- Include receipts and any required documents. Keep them for 12 months (for online claims), or include them with your mailed-in claim.
Claim forms:
Extended health claim form – For eligible prescription drugs, vision care, registered specialists or therapists, and medical equipment or rentals.
Dental benefit claim form –Your dentist or dental specialist must complete this form.

You can start your travel insurance claim online or send it to us by mail.
Online:
- Go to the GEM Travel Claims portal. Sign in with your email and password.
- Click New claim
- Click Add a new policy number. Type in your Manulife policy number.
- Choose your claim type from the list.
- Fill in your personal information and choose your contact reference.
- Complete the certification and payment section.
- Enter your claim details.
- Upload any receipts or documents required to support your claim.
- Click Submit to finish.
You’ll receive a confirmation email after you submit your claim. Once your case is being reviewed, a claims examiner will be in touch with you.

If you haven’t used tobacco, tobacco cessation products, or marijuana for 12 months in a row, you may qualify for lower premiums. If approved, your lower premium rate would start on your next payment date.
To apply, fill out the Change smoking status form (PDF) and mail it in to us.
To change your beneficiary, download the Beneficiary Designation (Affinity Markets) form. Complete the form and submit it to us at the address included at the top of the form. If you have any questions or concerns, please give the customer service centre a call at 1-800-268-3763 Monday to Friday, 8 a.m. to 8 p.m. ET.
