Change your beneficiary

When you purchase a life insurance policy, or join a retirement plan through work, you’re asked to name a beneficiary. At certain times in life, however (e.g., divorce, relationship changes), a change in beneficiary may be required.

To complete the change, based on your relationship with Manulife, choose from the listed categories below, then complete and provide us with the appropriate signed form.

(Note: the instructions below do not apply to all plans. If you are unable to update your information using the instructions below, call the Customer Service Centre for directions specific to your plan.)

To update the name of your beneficiary(ies), if your life insurance is with Manulife:

  1. Sign in to your plan on the website
  2. Click on the Group Benefits tile, then click “Go” under the plan you want to access
  3. Under “Forms,” click “Administration Forms – Find a form – Change of Beneficiary”
  4. Complete the form and submit it as per the instructions on the form

OR

  • Download the Beneficiary designation form (it may download to the left lower corner of your screen, depending on your browser settings). Save a copy to your computer, fill in the information, and send the completed form to us.

As a Group retirement plan member, to change, add or delete a beneficiary, you can:

  1. Sign in .
  2. Go to My Profile →  View My Member InformationBeneficiary Information section → Update

Otherwise ...

  1. Fill out a form for the plan you want to change your beneficiary for.
    Request for change - RRSP, TFSA, LIRA, RPP, NRSP
    Request for change - RRIF, LIF, LRIF, PRIF, RLIF
    Did you name an irrevocable beneficiary last time? Make sure they sign the form.
  2. Send us the form you filled out. Sign in and go to My AccountSend documents or use the mailing instructions on the form.

VIP Room 

  1. Sign in .
  2. Click VIP Room - Group Savings & Retirement or choose the program you want.
  3. Go to Manage your plans Personal data.

Otherwise…

  1. Fill out a Miscellaneous change form.
  2. If you named an irrevocable beneficiary last time, get them to sign a Waiver of rights as irrevocable beneficiary form. To find it:
    1. Sign in .
    2. Click VIP Room - Group Savings & Retirement or choose the program you want.
    3. Go to Plan your financial futureHelpful information.
  3. Use the mailing instructions on the form (or forms) you filled out, or send it to us online:
    1. Sign in .
    2. Click VIP Room - Group Savings & Retirement or choose the program you want.
    3. Go to Manage my plansSend documents.
